T2026-03 The 24 January 2026 death of a young person who had left a child welfare institution without permission in the Pirkanmaa wellbeing services county
The Safety Investigation Authority will launch a safety investigation into the 24 January 2026 death of a young person who had left a child welfare service unit without permission. The young person had been placed in a unit providing 24-hour child welfare substitute care services in the Pirkanmaa wellbeing services county.
The investigation will focus on modes of operation used to ensure the safety of young people placed in child welfare institutions as well as the cooperation practices between child welfare and other authorities related to situations in which a young person leaves the institution without permission.
Eero Nyman will be appointed as Head of the Investigation Team, and Senior Specialist Krista Lyyra, Specialist Salla Redlin and Specialist Mikko Salasuo will be appointed as its members. The Investigator in Charge is Hanna Tiirinki, Chief Safety Investigator.
