R2017-01 Decision to initiate an investigation of an incident that occurred in Ylivieska on 28 June 2017
On the basis of section 2 of the Safety Investigation Act (525/2011), the Safety Investigation Authority has decided to initiate an investigation of an incident that occurred in Ylivieska on 28 June 2017, in which a track tamping machine which had begun moving by itself ended up on a track used by traffic.
After work on the previous night, the track tamping machine, which had been used for track construction, had been left on a single cross-over between a line used by traffic and the line under construction, and the crew which had been operating it had departed. Around four hours later, the track tamping machine began moving of its own accord, trailed a switch onto a track used by traffic and ran for around a kilometre onto track 1 of the Ylivieska station, where it came to a halt. The engine driver of a passenger train standing on track 3 ensured that the tamping machine would stay in place.
Jussi Kangasmaa (MSc. in Engineering (Tech.)) is appointed as a team leader of the investigation team and Rail Safety Investigator Lasse Laatta, Ralf Sandberg (traffic controller (retired)) and Martti Peräaho (MSc. in Psychology) are appointed as members. The Investigator-in-charge is Esko Värttiö, Chief Rail Safety Investigator.
